How to Make a Referral

Referring persons who are blind and visually impaired for services is easy.
Just call toll-free in New Hampshire 1-800-464-3075.

The Referral Process

  1. The person making the referral will talk to a member of the Social Work Department.
  2. The Social Work staff will provide a brief overview of services available as well as obtain some initial information about the person with vision loss.
  3. Arrangements will be made for an in-home visit to the person with vision loss to further discuss services and jointly develop an Individual Service Plan according to the person's needs.
  4. An Eye report is needed to assist in the provision of services; the Social Work staff will assist in obtaining this information with permission of the person with vision loss.

Referrals are received by mail, or phone, from doctors, health care providers, family members, friends, neighbors, and the individuals themselves. No one is denied services due to the inability to pay.
